About the toolkit

This toolkit contains tissue viability improvement resources. These tools and techniques have been designed by experts in the area of pressure ulcer care.
Key principles of tissue viability online;

  • it is intended to help with the daily work of caring for the patient/client
  • it is simple to use
  • its content is designed to be accessible by you
  • it is aimed at NHS and care home staff in Scotland
  • each section can be easily identified
  • each tool has a clear purpose – to add value to your care.

The toolkit aims to help improve your skills in the care of patients/clients:

  • at risk of developing a pressure ulcer
  • with a pressure ulcer.

The available tools and resources work together like the structure of a temple.

The foundation

The ‘temple’ of improved care would collapse without its foundation. The foundation has 3 parts:

  • an educational workbook
  • the best practice statement
  • improvement methodology.

NES’ educational tool builds knowledge and understanding. NHS QIS’ best practice statement offers a practical how to guide based on evidence and professional consensus. The methodology used by the Scottish Patient Safety Programme (SPSP) gives you a simple approach to improving your patient/client care. Remember; start with small tests of change. Small improvements together can bring about big changes.

The temple pillars

With foundations in place, the pillars give specific tools to help you improve overall practice and care.

These are:

  • Risk assessment
  • Care bundle
  • Pressure ulcer grading
  • Excoriation
  • Wound Assessment
  • Pressure ulcer prevalence.

Remember, good care can prevent many pressure ulcers. To achieve a better care outcome, combine:
