A key issue in pressure ulcer recognition is differentiating between tissue damage resulting from pressure compared with other types of tissue damage which result from other causes such as exposure of the skin to excessive moisture. One indicator that can be used to assist with this is observing the anatomical area over which tissue damage has occurred as pressure ulcers usually occur over bony prominences.

Incontinence is recognised as a factor which can cause soft tissue damage by excoriating the skin through prolonged contact with urine and faeces. A prerequisite to pressure ulcer grading is to first of all correctly identify the cause of tissue damage in order that care may be planned appropriately.
