This chart should be used in combination with the grading tool and the excoriation tool to help you to determine the grade of pressure ulcer, then record each pressure ulcer and its grade on the chart. If you use one form per individual you will end up with an accurate assessment of pressure ulcers in each individual in an area.
If this chart is completed by a designated person in each area at a designated time and then the information from all the charts is collated, an accurate picture of the prevalence of pressure ulcers in that area can be developed.
The information from collated forms is linked to a data collection process provided by ‘Survey Monkey’ and is designed to contribute to a national prevalence study should this be instigated.
