Daily repositioning and skin inspection chart
This is a chart to help you keep track of how often and when you move the patient/client.
This is a simple form which you can keep with the patient's/client's 's notes and which can be completed at every repositioning. It should help you and others in the team keep track of your efforts to minimise the risk of a pressure ulcers developing through turning or repositioning the patient and thereby allowing different parts of the body in turn to be exposed to pressure.

  • Repositioning is only one way of minimising the risk of pressure ulcers. Use this means of reducing risk along with other means.
  • Remember the needs of the patient/client and reposition with sensitivity to the situation. For example, there may be other considerations for a patient/client receiving palliative care.
